Healthcare policy significantly develops the Advanced Practice Nurse (APN) profession in many ways. Healthcare policies dictate the educational requirements of APN programs and the scope of practice for the APN and influence the reimbursement rates for practicing APNs. The Advanced Practice Nurse has the educational requirements and scope of practice clearly defined by state legislation; for Florida, this would be under Florida Statute 464 (The Florida Senate, n.d.). The state policies also define the type of licensure available for practice: full practice, reduced practice, or restrictive practice. Florida has restrictive licensure requiring a collaborative agreement with a supervising physician until specific requirements are met to obtain autonomous practice licensure (Florida Board of Nursing, n.d.). Healthcare policy also influences reimbursement rates nationally for services in patient care. Nurse practitioners are reimbursed about 15% less than physicians for the same services rendered. This causes many NPs to bill indirectly under the physician, creating a needed collaboration instead of an autonomous practice (smith, 2020).

 

Advocacy in healthcare means to speak on behalf of another or to plead their case. Advocacy is the basis of nursing at all levels and is crucial for expanding the scope of practice and autonomy of APNs. Advocacy allows APNS to influence healthcare policies and advocate for the best interests of their patients. Nurses at every level are taught to be patient advocates, always trying to do what is best for the patient or what the patient desires. This allows the patient to receive quality patient-centered care while allowing autonomy for the patient to decide the direction their care will take. Nurses must also advocate for themselves and their profession. The APN role includes advocating for the advancement of the APN profession, improving the scope of practice, and autonomous licensing nationwide. Standardizing the licensing for autonomy will help decrease the shortage of primary care providers nationally (Jurns, 2019).

 

Transformational leadership is an engaging relationship between the leader and followers that instills trust, empowers the followers to be creative and innovative, and focuses on developing the team towards a common goal or purpose. There are four pillars of transformative leadership: professionalism, inspirational motivation, interpersonal relationships, and business skills (DeNisco & Barker, 2016). In terms of transformational leadership, professionalism is the ability to be self-aware of one’s strengths and weaknesses by reflecting on experiences and knowledge. Inspirational motivation is finding purpose and meaning in a shared vision. The leader welcomes innovation and encourages thinking outside the box to inspire and motivate the followers to be their best. Interpersonal relationships are positive relationships between all members with a shared power built on trust and respect. Lastly, business skills mean understanding the healthcare system and its parts: policy, finance, marketing, etc. Transformational leaders are charismatic, motivating, and able to clearly state their purpose and vision, inspiring others to work towards the same goal (Metz et al., 2019). This type of leader can affect change in policy by doing just that. They can rally others to join their fight in making positive changes. A transformational leader will be able to present a clear and thorough picture of the changes that need to happen and how these changes will benefit patients, lawmakers, and providers.
